OWN DAMAGE
pertains to damage on the insured vehicle caused by: a) the assured's own fault, b) thru the fault of a third party who refuses to assume liability, or c) intentional damage inflicted by an unknown party while the insured car is attended or unattended.
Documents to be submitted:
- Affidavit of Incident or Incident Report Form
- Photographs of: a) the damaged portion/s, and b) of a full view of the vehicle showing the plate number and if possible the damages
- Repair Estimate
- Photocopy of Driver's License Identity Card with OR
- Photocopy of vehicle CR and OR
THIRD PARTY PROPERTY DAMAGE
Pertains to damage caused by the insured vehicle on vehicular or non-vehicular property/ies owned by other parties and for which the insured is liable.
Documents to be submitted:
- Affidavit of Incident or Incident Report Form
- Photographs of: a) the damaged portion/s, and b) of a full view of the vehicle showing the plate number and if possible the damages
- Repair Estimate
- Photocopy of Driver's License Identity Card with OR
- Photocopy of vehicle CR and OR
- Original copy of Certification of No Claim from third party insurer (if third party Property Damage is a motor vehicle)
- Proof of Ownership (if third party Property Damage is not a motor vehicle)
THIRD PARTY BODILY INJURY
pertains to physical injuries caused by the insured vehicle on third party persons and for which the insured is liable.
Documents to be submitted:
- Police Report
- Affidavit/s of Incident or Incident Report Form
- Photocopy of Driver’s License Identity Card with OR
- Photocopy of vehicle CR and OR
Additional documents for Non-Death Claims
- Photocopy of valid government Identification (if of legal age) or photocopy of Birth Certificate (if minor)
- Original copy of Medical Certificate
- Original copy of official receipts of medical expenses with attending doctor’s prescription
Additional documents for Death Claims
- Photocopy of valid government Identification of claimant/heirs
- Photocopy of Birth Certificate
- Photocopy of Marriage Certificate (if applicable)
- Certified true copy of Death Certificate
- Original copy of funeral bills/official receipts
CARNAPPING/TOTAL LOSS
pertains to loss of insured vehicle or a part of the vehicle thru theft.
Documents to be submitted:
- Police Report
- Affidavit/s
- Photographs of: a) vehicle portion/s affected by theft, and b) of the full view of the vehicle showing the plate number and if possible portion of vehicle affected by theft
- Cost estimate for replacement of vehicle part/s affected by theft (in case of theft)
- Photocopy of Driver’s License Identity Card with OR
- Photocopy of vehicle CR and OR
- Original copy of Complaint Sheet from the Traffic Management Group (TMG), in case of carnapping
- Original copy of Alarm Sheet from TMG (in case of carnapping)
- Original copy of Certificate of Non-Recovery from TMG (in case of carnapping)
- Full set of keys to vehicle
- Full set of keys to vehicle
- Letter of Release from financial institution (if applicable)
- Original of Cancellation of Chattel Mortgage (if applicable)
- Secretary’s Certificate authorizing the person who will sign the Deed of Absolute Sale and Release of Claim (only in the case of corporations)
PFIC reserves the right to request additional documents as it deems needed to facilitate processing of the claim

Who is a Third Party?
Third party person refers to any person EXCEPT the following:
- the insured
- a passenger as defined by law
- a member of the assured's household
- an employee of the assured
- any of the assured's family member up to the 2nd degree of consanguinity (i.e. parents, grandparents, siblings, children & grandchildren) or by affinity (parents-in-law, siblings-in-law, adopted children & children of adopted children)
